We accept Blue Cross/Anthem PPO, Aetna PPO, United Healthcare PPO, Cigna PPO and Blue Shield PPO.

We do not accept PHCS plans or any HMO plan.

Please state that you are Dr. Crystal’s patient when leaving voice mail messages as Virginia does the billing for several physicians.
